# Austin hospital ownership census: appendix

**Research cutoff: October 30, 2026. Geography: Travis, Williamson, Hays, Bastrop and Caldwell counties.**

This is a reconciled roster of **48 records: 42 general/special hospital license-location entries, five private psychiatric entries and Austin State Hospital.** It also identifies **Ascension Seton Bastrop, a known department of another hospital, separately below**. The 47 private records represent **40 distinct state licenses and 37 distinct nonblank Medicare certification numbers (CCNs)**. All 37 CCNs match CMS enrollment records. These are not 48 independent businesses, reporting entities or necessarily physically separate campuses. Texas NeuroRehab has two licenses on one site; several other campuses share a license and CCN.

The state directories are internally dated October 21, 2026; CMS ownership/enrollment files are dated September 29, 2026. A current directory entry establishes inclusion in the state roster, not a separately checked real-time operating license status. Addresses, legal operators, source links, raw classifications and qualifications are retained in the companion CSV/JSON. Ordinary freestanding emergency departments are outside this inpatient-hospital roster; microhospitals on the general-hospital list remain included.

**Source basis for all tables:** [HHSC general/special directory][general], [HHSC private psychiatric directory][psych], [CMS hospital enrollment][enroll] and [CMS hospital owners][owners]. Blank CCN cells mean no CCN in the HHSC row, not a finding that the facility is unregulated. Nonprofit/proprietary labels concern the recorded operating entity; none of these labels by itself proves a property-tax exemption.

## Ascension Seton: 11 roster entries

Ascension is the Catholic nonprofit parent; ASCENSION SETON is the CMS operating entity for ten rows. **Cedar Park is the exception:** its operator is CEDAR PARK HEALTH SYSTEM LLC, still recorded as proprietary by CMS and for-profit by HHSC. CMS reports Ascension Texas at 99% direct and CP Hospital GP LLC at 1%, effective June 30, 2025. Dell/UT names identify donor and academic relationships; they do not establish that UT operates the hospital. Ascension’s stakes in other operators appear in the joint-venture section.

**Known department outside the standalone roster:** Ascension Seton Bastrop, 630 SH 71 Service Road, became a department of Ascension Seton Hays on **October 1, 2024**, according to Seton’s FY2025 Schedule H; former license 100497. **Identifier change:** Smithville’s current critical-access CCN is 671304. The prior September 2024 enrollment used CCN **450143**; use that historical identifier for its FY2024 cost report. ## St. David’s HealthCare: 8 roster entries

All eight rows resolve to **ST DAVIDS HEALTHCARE PARTNERSHIP LP LLP**, the HCA/foundation partnership. The foundations’ 2024 tax returns disclose **St. David’s Foundation 40.59% and Georgetown Health Foundation 1%**; HCA’s exact current interest is not independently verified and is not inferred by subtraction. These economic shares do not establish governance votes. Baylor Scott & White Health is the nonprofit parent. **Round Rock, Lakeway and Cedar Park Emergency Medical Center share SCOTT & WHITE HOSPITAL–ROUND ROCK, one state license and one CCN.** Austin, Buda and Pflugerville share the legal entity BAYLOR SCOTT & WHITE MEDICAL CENTERS–CAPITOL AREA but have distinct CCNs; Taylor is a separate legal entity/CCN. The BSW-branded rehabilitation hospital below is a different, proprietary joint venture.

## BSW / Select Medical rehabilitation joint venture: 1 entry

The proprietary operator is **BIR JV LLP**. CMS reports Baylor University Medical Center’s 51% limited-partnership interest, Select Physical Therapy Texas LP’s 49% direct interest and Select Medical’s operational control. **Select Medical itself came under a WCAS-led private-equity consortium with Robert Ortenzio and Martin Jackson on July 1, 2026.** WCAS says the consortium controls operations and holds a majority of Select’s economic interest. That is upstream of Select’s 49% hospital-JV interest; it does not turn the Baylor partner’s 51% into WCAS ownership. Exact consortium/fund stakes are not disclosed. ## Scope and ownership claims that need restraint

1. **Roster completeness is not campus completeness.** All rows in the two selected HHSC private-hospital directories for the five counties are accounted for, with the state hospital added. A complete service-site inventory would additionally require all hospital departments and satellite locations; Bastrop is one documented extra. St. David’s Children’s Hospital/Women’s Center brands are within North Austin Medical Center and are not extra independent roster entries.
2. **A closed license is not necessarily a closed care site.** Bastrop became a department. Conversely, Ascension Seton Shoal Creek provided no services after May 31, 2025, according to Seton’s FY2025 filing, and is excluded from the current roster. [Seton filing][seton]
3. **Parentage, entity classification, property title and exemption are separate fields.** Six proprietary hospital operators have documented Ascension stakes: Cedar Park, Central Texas Rehabilitation, Cross Creek, Northwest Hills Surgical, PAM Kyle and PAM Round Rock. The BSW/Select rehabilitation JV is also proprietary. This does not establish which individual parcels are taxed or exempt.
4. **CMS owners are not a ready-made cap table.** The file includes direct owners, indirect owners, general partners, officers, managers and secured creditors. Some percentages occur on non-equity roles; some historical association records overlap. No ownership tree should add all rows or interpret a lender’s security interest as equity.
5. **Financial joins must use both entity and date.** One CCN can cover multiple campuses; one tax return can cover multiple CCNs. Smithville changed identifiers. Cedar Park’s 2025 acquisition, Cross Creek’s 2025 partnership and Select’s 2026 privatization must not silently rewrite 2024 financial ownership.

**Unresolved ownership cases:** the current exact Lifepoint/Apollo fund interests; Scion/Knight’s ultimate funds; PAM’s complete Misitano/fund cap table; Signature’s exact Kim/family stakes; WCAS/Ortenzio/Jackson’s exact Select consortium percentages; Northwest Hills’ remaining current minority interests and managerial allocation; complete Attila/Welch/physician stakes; Family Hospital Systems’ ultimate interests; Kyle ER’s legal owner/ultimate parent; and the separate NeuroRehab psychiatric licensee. Each is marked in the machine-readable census rather than filled with an assumed parent.

**County reconciliation:** Travis 24 + Williamson 16 + Hays 5 + Caldwell 2 + Bastrop 1 = **48 roster/state records**. Add Bastrop’s documented department as a separate care-location line, not as another independent license or CCN.
